EleVis Wellness Collective is seeking a motivated Physical Therapist to join our team in Chicago. In this role, you'll provide hands-on, patient-centered care in a supportive wellness-focused practice, working alongside a collaborative team dedicated to helping clients move better, recover fully, and feel their best. This position is available full-time or part-time and is a great fit for both experienced clinicians and new graduates eager to grow their skills, including training in integrative treatment modalities.

Requirements

Education: postgraduate degree

Qualifications:

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Current state licensure as a Physical Therapist in good standing
Individual NPI
Strong knowledge of musculoskeletal rehabilitation, biomechanics, and movement science
Experience with manual therapy and functional rehabilitation preferred
Willingness to learn and be trained in additional integrative modalities

Skills:

Strong clinical reasoning and patient assessment abilities
Comprehensive musculoskeletal and functional movement evaluation skills
Postural and biomechanical analysis expertise
Ability to identify mobility restrictions, muscle imbalances, and movement dysfunction
Differential diagnosis of orthopedic and neuromuscular conditions
Excellent communication and patient engagement skills

Responsibilities:

Perform comprehensive patient evaluations and develop individualized treatment plans
Design and progress rehabilitation programs based on patient needs and goals
Educate patients on movement, recovery strategies, and injury prevention
Collaborate with a multidisciplinary team to ensure coordinated care
Deliver patient-centered, outcomes-driven therapy services
Maintain accurate and timely documentation of patient progress
Assist patients with exercises, mobility training, and therapeutic activities
Safely support or lift up to 40 lbs when necessar
